A ratcheted handle for 5 mm laparoscopic inserts. The ratchet holds grip pressure without the surgeon maintaining it, which is what makes it the right choice for retraction and any hold that has to last — a plain handle turns a long hold into an endurance exercise.
The No-10 pattern places the ring lower relative to the shaft axis than the No-3 or No-7, giving a more neutral wrist angle when working towards the surgeon. Which pattern suits comes down to build and preference, so most theatres end up stocking two or three.
The insert collet accepts the full range of graspers, dissectors and scissors, releasing with a quarter-turn for cleaning. The ratchet disengages with a thumb movement without letting go of the handle.

Should I buy ratcheted or plain?

ARatcheted for grasping and retraction, where holding pressure for long periods is tiring. Plain for dissection, where you want the jaw to release the instant finger pressure comes off. Most theatres stock both.

AThey differ in where the finger ring sits relative to the shaft axis. The No-7 is more in-line for straight-on approaches; the No-10 sits lower for a more neutral wrist when working towards yourself. Which suits comes down to hand size and preference.

ANo — the handle is sold on its own and inserts are ordered separately. This is deliberate, since most theatres want one or two handles and many different inserts.
