How to Autoclave Laparoscopic Instruments Without Shortening Their Life
Most instrument damage happens in the sterile services department, not the operating theatre. Here is what actually shortens instrument life, and how to avoid it.

Ask a theatre manager where instruments get damaged and most will say theatre. In practice, the majority of damage happens between cases — in cleaning, in packing, and in the autoclave itself.
Clean before you sterilise
An autoclave sterilises; it does not clean. Protein and blood residue baked onto a jaw at 134°C is considerably harder to remove afterwards, and it shields the surface underneath from steam. Enzymatic cleaning first, always, and within twenty minutes of use where possible — dried blood is the single most common cause of pitting.
Open every hinge and strip every channel
A closed ratchet does not get sterilised on its bearing surfaces, and a trocar with its valve housing still assembled traps residue in exactly the place you cannot see. Instruments that come apart are designed to come apart. If your set includes an instrument nobody strips because it is fiddly, that is the instrument that will fail first.
Do not stack
Steam has to reach every surface. Instruments piled on each other in a solid tray block circulation and trap condensate, which is why loads come out wet and occasionally fail their sterilisation check. Perforated trays and silicone finger mats exist for this reason, and they pay for themselves in avoided reprocessing.
Let it cool on its own
Quenching a hot instrument to speed up turnaround is the fastest way to destroy the seals in a trocar valve or the bonding on a handle. Thermal shock also encourages micro-cracking at welds. Let the cycle finish its drying phase.
Watch the water
Hard water leaves mineral deposits that look like staining but behave like abrasive. If your instruments are developing a persistent white film, the problem is the water supply to the autoclave, not the cleaning protocol.
Inspect before packing, not after unpacking
A chipped jaw or a sprung ratchet found on the tray in theatre costs you a case delay. Found in CSSD, it costs you nothing but a swap. Five seconds per instrument at packing is the cheapest quality control available.
